Continuing Health care - When a young person with SEND has a primary health need or extreme behaviour that challenges and is eligible for NHS Continuing Healthcare, the NHS is responsible for commissioning a care package that meets their assessed health and associated social care needs.This session will focus specifically on the CHC assessment process and will include an explanation of CHC, the eligibility criteria, timescales and pathways and an explanation of the forms and processes for those young people moving into adult services. Vicky Cameron, Children’s Continuing Care Health Assessor, will  be attending from CCC to discuss her role in transition from children’s continuing care (CCC) to adult Continuing heath care (CHC).
